Resumo
A estrutura de Design de Transição (TD) exige a integração de teorias de mudança ao projetar mudanças em nível de sistemas. Enquanto isso, uma teoria da mudança descreve a relação entre as ações tomadas e os resultados obtidos no processo de iniciar a mudança.
